- First of all, you have to visit the official website of the SSEPD portal.
- Here you have to click on the "PWD ID Card" button. it takes you to the next page.
- Now, you can see two options on your screen. If you are applying for yourself then you have to select the "Person with Disability" button. If you are applying for children then you have to select the "Children with Disability".
- Now, you have to click on the "Apply for " button. It takes you to the Identity Card Application form page.
- So now, you can see 4 sections of the PWD Application form. The first section contains personal information of candidate. Here you have to enter applicant name, father name, mother name, date of birth, gender, and mobile number.
- in the bottom, you have to select the category, blood group, marital status. After that, enter UDID Application Number and Date. Later you have to enter the address of the applicant.
- In the second section, you have to enter the PWD Disability Details. Now, select yes on the "Disability Certificate". After that, select the disability Type and Disability Since or Disability by birth. Later, you have to select Disability, Hospital treating disability, etc.
- In the third section, you have to enter your employment detail like employed or not, occupation, unemployed since, etc.
- In the last section, you have to upload your identity proof document like Aadhaar number. Now, you have to click on the "Apply" button.
- Finally, you have successfully applied for Odisha Disability Identity Card Online.
How to Check Odisha PWD ID Card Application Status
- First of all, you have to visit the official website of the SSEPD portal.
- Here you have to click on the "PWD ID Card" button. it takes you to the next page.

- Here you have to click on the "Search PWD Application" button. it takes you to the Application Status page. Now, enter your Application number and you can see the status of your application on your screen.

- Here you have to click on the "Search PWD Application" button. it takes you to the Search Odisha PWD Beneficiary page.
- So now, you have to select the name or aadhaar in "Search By". After that, you have to enter the name or aadhaar number and click on the Search button. It shows you a list of PWD persons who have applied for the identity card.
- If you want to update details click on the eye icon below view or update details column. if you want to view the application form click on the icon under View application form column.
